competency level.
2. Obtain a provisional permit from the ISDH once admitted into the program.
3. Complete an approved education program (fulfilled by this course).
4. Concurrently complete clinical training in a dental office with a
licensed dental radiographer..
5. Upon completion of the educational program (this course) and the clinical
training in an office, submit the "Certificate of Proficiency for Limited
Radiographer" form available at
http://www.in.gov/icpr/webfile/formsdiv/53194.pdf to the Indiana
State Department of Health to receive eligibility to take the Dental

followed by an individual
