Professional Staff Search and Screen Guidelines


A "Personnel Appointment Request" and "Position Requisition" form should be completed and sent to Human Resources, who will forward it to the appropriate administrators for approval. Copies of the form are available on the share drive.

Once the completed forms are received, the Director of Human Resources and the Director of Affirmative Action will issue an "Authority To Recruit" memorandum outlining the appropriate hiring procedures.

Applicant Monitoring Form

The Applicant Monitoring Form must be sent along with the form letter selected.
Directions for completing the form:

The Hiring Department should send the Applicant Monitoring Form to applicants via e-mail as an attachment with these directions:
Use the mouse to click on or tab to the gray areas on the form to complete.
Be sure to complete questions 2 and 3.
Save the form as a document and send it as an attachment to aaoffice@iusb.edu



The Hiring Department will choose the correct form letter to send to the applicant to accompany the Applicant Monitoring Form from the list below.
